Your New York Medicare Review Checklist
A productive Medicare conversation starts with the questions you want answered. This checklist helps you prepare for a phone or video appointment without sending sensitive documents through a general web form.
Gather basic coverage information
- The name of your current coverage and when it began.
- Any notice that employer or other coverage is ending.
- Your New York ZIP code and county.
- Questions you have about enrollment timing.
You do not need to put your Medicare number or Social Security number into the contact form on this site. Discuss any later documentation request directly with the person helping you.
List your care priorities
Bring the names of doctors and hospitals you prefer, plus medications and pharmacies for a private review. Note whether you regularly travel or spend part of the year elsewhere. Explain any difficulty you have had understanding a bill or getting care so the discussion addresses your actual concerns.
Ask for clear comparisons
- How would I use my usual doctors under this option?
- What would I pay monthly and when I receive care?
- What limits or approval requirements should I understand?
- What enrollment window applies to me?
- Where can I read the official plan documents?

Before ending the conversation, make sure you understand whether you are only gathering information or taking an enrollment step. Ask what remains to be verified. Submitting our review request does not change your current coverage.
