This notice describes the current ClearHarbor Medicare website, focused on Medicare inquiries from New York residents.

Information collected through the form

The review form collects first and last name, email, phone, ZIP code, your selected coverage situation, and permission to contact you. Submitted requests are saved in the website’s hosted database together with the consent wording, source, and submission time. These details are used to handle your Medicare inquiry.

Appointments and service providers

Appointment buttons take you to Calendly. Calendly processes information you provide there under its own privacy policy. Website hosting and security services may process technical information such as IP addresses, browser details, request logs, and security cookies. Automatic Constant Contact enrollment is not currently connected.

Your choices

Submitting a review request does not enroll you in insurance or change your existing coverage. Do not enter Medicare numbers, Social Security numbers, or medical records into the general contact form.
